The Heatmiser EP Yellow No. 5 was released on 4/26/94 according to Wikipedia. I've also seen it listed elsewhere as being released on 5/13/94 but I'm not positive which is correct.
The EP (other than the song Wake which was recorded 11/92) was recorded in February 1994. According to the liner notes, it was recorded at C.A.M.M.

My understanding is that it was only put out on CD at this time. Vinyl copies were put out in future years but there are some photos of them here.
