USA CD [Frontier 31062-2]
This EP was only put out on CD upon its initial release in 1994. This is the US pressing on Frontier. It comes with a four panel booklet. The inner pages of the booklet have the song lyrics. The back panel of the insert has a picture of the band in front of shelves of records. The credits state that the cover illustration is by Colin Burns. Photographs are by JJ Gonson, computer assistance by Chris Johnson, and art direction by Neil Gust. Barcode according to Discogs is 0 1866-31062-2 9.

USA 10" [Frontier 31062-1; clear vinyl reissue]
Yellow No. 5 appears to have been reissued by Frontier on four different versions of colored vinyl. This is the clear vinyl version and it came out in either 2006 or 2007. Punkvinyl.com has an article about it being new in April of 2007 but perhaps it actually had come out a few months earlier in 2006, especially since 2006 is listed on the back cover. It includes an insert with lyrics and the photo that was on the back page of the CD insert. The artwork is slightly cropped compared to the original CD pressing, which can be noticed especially by one eye being slightly cut off from the face on the painting. The back photo is also slightly cropped. The front shrink wrap had a circle sticker stating "Special Colored Vinyl". The labels and back cover mention both the original release year of 1994 and also 2006. Barcode according to Discogs is 0 18663 10621 2.

USA 10" [Frontier 31062-1; purple vinyl reissue]
Yellow No. 5 appears to have been reissued by Frontier on four different versions of colored vinyl . This is the translucent purple vinyl version. It lists a date on the back cover of 2006 but punkvinyl.com mentioned it being a new color in 2009. So perhaps the 2006 date on the back was just kept from the clear vinyl (entry above). It includes an insert with lyrics and the photo that was on the back page of the CD insert. The front shrink wrap had a circle sticker on it saying "Special Colored Vinyl". The labels and back cover mention both the original release year of 1994 and also 2006. See clear vinyl entry above for a few more details. Barcode according to Discogs is 0 18663 10621 2.
USA 10" [Frontier 31062-1; blue vinyl reissue]
Yellow No. 5 appears to have been reissued by Frontier on four different versions of colored vinyl. This is the blue vinyl version. It still lists 2006 on the back cover but that may have just been the same cover used from the clear vinyl reissue. Punkvinyl.com has an article in 2015 mentioning this being a new color so it may actually be from 2015.

USA 10" [Frontier 31062-1; yellow vinyl reissue]
Yellow No. 5 appears to have been reissued by Frontier on four different versions of colored vinyl . This is the yellow vinyl version. All colors have a sleeve where the back cover lists 2006 on it but it appears that they were not all really pressed in 2006. Punkvinyl.com had articles when a lot of the other colors were released but I didn't see one specifically for this yellow vinyl, so I'm not sure of the exact year of release for this one.
USA 10" Test Pressing [Frontier 31062-2]
This is the test pressing for one of the 10" reissues on Frontier. Not much info is on Discogs for this pressing other than that it is in a generic sleeve and the barcode is 0 1866-31062-2 9.
USA Promo Split Cassette [Frontier ADV 31062-4]
This is a very interesting promo advance cassette for the Yellow No. 5 EP. Side one of the cassette has the full Heatmiser EP plus four songs from the upcoming album by The Blackeyed Susans. Side two is the remaining six Blackeyed Susans songs. It comes in plain white packaging with black writing and lists the street dates for each release.

USA Cassette [Frontier/Burger 31062-4/ BRGR657; 2014 reissue]
This is a 2014 cassette reissue that was put out by Burger and Frontier Records. It was a Record Store Day exclusive release and limited to 500 copies. It has the complete EP on both sides. The j-card pretty much has all the same info (one photo and lyrics) that the CD booklet has. Barcode according to Discogs is 634457649842.

Canada CD [Frontier 31062-2]
This Canadian pressing appears to be mostly identical to the original US CD pressing. The matrix number looks to be exactly the same as the US CD. Not exactly on the matrix, but on an even inner more circle part of the CD, near the matrix, this pressing is missing the code IFPI810A that is on the US pressing. This pressing has a "Made in Canada" sticker on the back of the jewelcase so presumably it's actually a Canadian pressing but US pressings of some other releases seem to have Made in Canada on them so I'm not positive that this is different than the US pressing. (The back tray of the CD packaging on my copy is white, as opposed to black on the US pressing, but I am not sure if the white tray is the original one or not since my copy was not bought brand new). Barcode and matrix according to Discogs:
