Most places online list the A Slice of Lemon compilation as being released sometime in 1995 but without a specific date. However, rateyourmusic.com lists it as being released on 5-12-95 so I'm going with that unless I hear differently.
A Slice of Lemon was released on both LP and CD and was a co-release by Kill Rock Stars and Lookout! Records. It has many, many songs on it including "Big Decision" by Elliott Smith and this was the first place this song was ever released. The CD came with a normal sized CD booklet while the LP booklet is very large like a magazine. It has credits for a lot of songs but Elliott's section just has a picture of him and his name (see photos below). According to the Alphabet Town site, the song was recorded in January-February 1995 at Leslie Uppinghouse's house. The LP is a double LP and the CD is a double CD.
The review of the album below is from the Spring 1996 issue of Puncture.
